Reminder: HIPAA format 5010 is proceeding. Don’t forget: The billing provider address, box 33 , and
the facility address, box 32 , must contain a nine digit zip. Claims with a 5 digit zip will reject at the Clearinghouse level.
That’s nine digits only, no spaces, no dashes.
Also, you must not use a PO Box in the facility and billing address field. Street address only.
